(A) Staff shall place infants up to 12-months of age on their backs for sleeping. This requirement is not met as evidenced by: Based on observation & record review, the licensee did not comply with the section cited above as infants under the age of 12 months are being placed on their stomach for sleep which poses an immediate health, safety or personal rights risk to persons in care.
(f) Cribs shall be free from all loose articles and objects, including blankets and pillows. This requirement is not met as evidenced by: Based on observation, the licensee did not comply with the section cited above as two infants were covered with a blanket while sleeping which poses/posed a potential health, safety or personal rights risk to persons in care.
(f) Cribs shall be free from all loose articles and objects, including blankets and pillows. (3) There shall be no objects hanging above or attached to the side of the crib. This requirement is not met as evidenced by: Based on observation, the licensee did not comply with the section cited above as five cribs had mobiles attached which poses/posed a potential health, safety or personal rights risk to persons in care.
(B) Staff shall physically check on sleeping infant(s) every 15 minutes and document the following: This requirement is not met as evidenced by: Based on interview & record review, the licensee did not comply with the section cited above as facility does not document 15-minute checks for infants 13 months of age and older which poses/posed a potential health, safety or personal rights risk to persons in care.
(1) On or before March 30, 2018, a person who, on January 1, 2018, is a licensed child day care provider, administrator, or employee of a licensed child day care facility shall complete the mandated reporter training provided pursuant to paragraphs (2) and (3) of subdivision (a), and shall complete renewal mandated reporter training every two years following the date on which he or she completed the initial mandated reporter training. This requirement is not met as evidenced by: Based on record review, the licensee did not comply with the section cited above as 3 staff members did not have a current Mandated Reporter Training Certificate which poses/posed a potential health, safety or personal rights risk to persons in care.
(e) In centers where the licensee chooses to handle medications: (3) Prescription medications may be administered if all of the following conditions are met: (B) For each prescription medication, the licensee shall obtain, in writing, approval and instructions from the child's authorized representative for the administration of the medication to the child. This requirement is not met as evidenced by: Based on record review, the licensee did not comply with the section cited above as two children do not have a signed consent form for their presciption medication at the facility which poses/posed a potential health, safety or personal rights risk to persons in care.
(B) For each nonprescription medication, the licensee shall obtain, in writing, approval and instructions from the child's authorized representative for the administration of the medication to the child. This requirement is not met as evidenced by: Based on record review, the licensee did not comply with the section cited above as two children do not have a signed consent form for their nonpresciption medication at the facility which poses/posed a potential health, safety or personal rights risk to persons in care.
