(b) All licensed Child Care Centers operating in buildings constructed before January 1, 2010, shall test their water for lead pursuant to these written directives. This requirement is not met as evidenced by: Based on the licensee statement and record review, the licensee did not comply with the section cited above, which poses a potential health, safety or personal rights risk to persons in care.
(b) The floors of all rooms shall have a surface that is safe and clean. This requirement is not met as evidenced by: Based on LPAs observation, the licensee did not comply with the section cited above in Room 6 as carpet needs to be cleaned: stained in several areas of the carpet which poses a potential health, safety or personal rights risk to persons in care.
(1) On or before March 30, 2018, a person who, on January 1, 2018, is a licensed child day care provider, administrator, or employee of a licensed child day care facility shall complete the mandated reporter training provided pursuant to paragraphs (2) and (3) of subdivision (a), and shall complete renewal mandated reporter training every two years following the date on which he or she completed the initial mandated reporter training. This requirement is not met as evidenced by: Based on record review, the licensee did not comply with the section cited above for 3 of the staff ( #1, #3 and #4) which poses a potential health, safety or personal rights risk to persons in care.
(1) Commencing September 1, 2016, a person shall not be employed or volunteer at a day care center if he or she has not been immunized against influenza, pertussis, and measles. Each employee and volunteer shall receive an influenza vaccination between August 1 and December 1 of each year. This requirement is not met as evidenced by: Based on record review, the licensee did not comply with the section cited above in 2 of the staff (#3 and #4) which poses a potential health, safety or personal rights risk to persons in care. Staff #3 is missing Measles and Pertussis, Staff #4 missing Measles.
(g) The licensee shall document each child's immunizations and shall maintain such documentation in the center for as long as the child is enrolled. This requirement is not met as evidenced by: Based on record review, the licensee did not comply with the section cited above in 2 of the children (Child #1 and Child #4). Child # 1 missing immunization records and physician report and Child # 4 missing Immunization records which poses a potential health, safety or personal rights risk to persons in care.
BUILDINGS and GROUNDS: Free of hazards including, but not limited to, holes, broken glass and other debris, and dry grasses that pose a fire hazard. This requirement was not met as evidenced by: jutting nails and ripped artificial grass were observed. This poses an immediate risk to Health and Safety to children in care.
PERSONNEL REQUIREMENTS: Request a transfer of a criminal record clearance. This requirement was not met as evidenced by: LIC 9182 was in the file but staff was never associated to the facility. This poses an immediate risk to Health and Safety to children in care.
MANDATED REPORTER TRAINING - This requirement was not met as evidenced by: director has not renewed the Mandated Reporter Training. This poses a potential risk to Health and Safety to children in care.
OUTDOOR ACTIVITY SPACE: As a condition of licensure, the areas around and under high climbing equipment, swings, slides and other similar equipment shall be cushioned with material that absorbs falls. This requirement was not met as evidenced by: insufficient resilient materials around and under the play structure. This poses a potential risk to Health and Safety to children in care.
OUTDOOR ACTIVITY SPACE: Sandboxes shall be inspected daily and kept free of hazardous foreign materials. This requirement was not met as evidenced by: sandbox was not covered. This poses a potential risk to Health and Safety to children in care.
