(1) On or before March 30, 2018, a person who, on January 1, 2018, is a licensed child day care provider, administrator, or employee of a licensed child day care facility shall complete the mandated reporter training provided pursuant to paragraphs (2) and (3) of subdivision (a), and shall complete renewal mandated reporter training every two years following the date on which he or she completed the initial mandated reporter training. This requirement is not met as evidenced by: Based on observation, interview and record review, the licensee did not comply with the section cited for one staff (S4) which poses a potential health, safety or personal rights risk to persons in care.
(1) Commencing September 1, 2016, a person shall not be employed or volunteer at a day care center if he or she has not been immunized against influenza, pertussis, and measles. Each employee and volunteer shall receive an influenza vaccination between August 1 and December 1 of each year. This requirement is not met as evidenced by: Based on observation, interview and record review, the licensee did not comply with the section cited above for two staff (S1 and S3) which poses a potential health, safety or personal rights risk to persons in care.
(3) Prescription medications may be administered if all of the following conditions are met: (B) For each prescription medication, the licensee shall obtain, in writing, approval and instructions from the child's authorized representative for the administration of the medication to the child. This requirement is not met as evidenced by: Based on observation, interview and record review, the licensee did not comply with the section cited above for one child (C7) which poses a potential health, safety or personal rights risk to persons in care. Director did not obtain written consent from parent or authorized representative to administer EpiPen to child.

This poses a potential risk to children's health and safety of children in care.
Health-Related Services:In centers where the licensee chooses to handle medications: Prescription medications may be administered if all of the following conditions are met: Prescription medications shall be administered in accordance with the label directions as prescribed by the child's physician. This was not met by: LPA's observed that C1 Epi-Pen expired 4/2021.This poses a potential risk Health, Safety Personal Rights risk to children in care.
