(1) On or before March 30, 2018, a person who, on January 1, 2018, is a licensed child day care provider, administrator, or employee of a licensed child day care facility shall complete the mandated reporter training provided pursuant to paragraphs (2) and (3) of subdivision (a), and shall complete renewal mandated reporter training every two years following the date on which he or she completed the initial mandated reporter training. This requirement is not met as evidenced by: Based on record review, the licensee did not comply with the section cited above in 2 out of 2 adult helpers which poses a potential health, safety or personal rights risk to persons in care. Licensee's adult helpers (A1 & A2) have expired Mandated Reporter Training.
(d) For a Large Family Child Care Home, the maximum number of children for whom care may be provided at any one time when there is an assistant provider in the home, including children under age 10 who reside at the licensee's home and the assistant provider's children under age 10, shall be either: (1) Twelve children, no more than four of whom may be infants; or This requirement is not met as evidenced by: Based on LPAs observation and licensee's statement, the licensee did not comply with the section cited above by having 10 children including five (5) infants and five (5) preschool age. This poses an immediate health, safety or personal rights risk to persons in care.
