It was determined during the investigation that the operation did not report an incident in the required time frame to licensing. This was corrected at Inspection when we notified the director that they have 48 hrs to report an incident that they become aware of.
There was no documentation of administering medication training for staff, although there are children in care requiring medication to be administered.
There was no documentation of the operation inspecting fire extinguishers, smoke detector or carbon monoxide detectors. This was corrected at inspection when the director tested all smoke detectors, carbon monoxide detector and fire extinguishers and documented the information on form 7263.
I observed expired medication authorizations for children requiring administration of medication.
There was expired medication stored in the medication lock box. All stored medication was sent home with the appropriate child.
The operational policy was lacking information regarding the following; - parent notifications - suspension and expulsion - infant safe sleep - policy changes - promotion of indoor/outdoor activities including benefits and duration - parent communication with director - emergency preparedness plan - nursing mothers - health checks (if applicable) - employee vaccinations (if applicable) - procedures for inclusive service to children with special care needs - gang- free zone information - transportation, water activities, field trips, and animals (if applicable).
A caregiver requiring SIDs had an expired SIDs certification.
The stored medication was not in the original container. All stored medication was sent home with the appropriate children.
Observed the last fire inspection completed on 9/22/2021. The operation will request for a new inspection to be completed.
Observed the operations fire extinguishers had not been serviced since April 2021. I spoke to the operation owner about completing this once a year.
