A review of the Employees Who Left Employment in 2021 data from February 23, 2022 revealed that the center had not reported the number of employees who left in 2021 by February 22, 2022, as required.
The operation's menu was not posted at the operation. This was corrected at inspection when the Director printed out the last two weeks of menus.
6 children's files were inspected. One child needed an up to date immunization record. Three children needed an emergency contact address. Two children needed health statements. Four children needed Dr. and Hospital address and phone numbers for the authorization for emergency care.
The Director's background check showed as inactive during the inspection.
Paint located close to the floor in a classroom was tripping and peeling. Metal poles for a gate were sticking out of the ground outside on the playground. The carpeting under the rocks outside was coming up and creating a fall hazard.
