It was observed that the trash can in the school age room and kitchen that was filled with food and garbage. A trash can lid in the kitchen had visible dirt and food stuck on to it. The rolling chair in the preschool room has dirt and stains visible. There are holes and paint peeling in the walls of the school age room. There is visible dirt on the walls and floors throughout the operation. There was sink of dishes, dirty plate with food and an open box of food on the counter in the kitchen. The contents of the fridge were dumped out on the floor. The sanitation inspection is past due. 1/3/2019
There was a mop bucket filled with soapy water in the toddler room. An electrical outlet was observed uncovered in the entrance area of the operation. There was a hands sanitizer bottle and mosquito spray on the toddler playground. There were 2 wet socks, pieces of trash and a wet plastic bag on the large playground. There was a lawn mower and gas can in the extra room where cots are stored and children can access. There are boxes in the walk area accessing the second bathroom. The anuunal fire inspection is past due dated 12/3/2018
At inspection, the operation did not have a current fire inspection.
At inspection, the operation was serving vanilla wafers and oatmeal cookies as part of an afternoon snack. This was corrected at inspection when the operation substituted vanilla wafer and oatmeal cookies, to graham crackers and whole wheat pretzels.
At inspection, three classrooms used, did not have an evacuation plan posted. This was corrected at inspection when the operation posted an evacuation plan in each classroom.
At inspection, the operation did not have a current annual sanitation inspection posted.
Seven spots of peeling paint are present in the entryway hall. There is discolored spot about the size of a dinner plate on the ceiling that appears to be moldy.
A playscape on the playground has a ladder in the use zone of one of the slides. It also has a platform that is too tall for the age of children that use it. The operation decided that they will just no longer use this section of their outdoor space.
Health statements for enrolled children were not available for review.
Two staff members have background checks that required re-checking as of 11-6-2023. There are two other staff members that will also require resubmitting that are not here today. These teaches will also not be able to be present at the operation until they have received an eligible status. The two present today have been told and asked to leave the operation. I watched them leave.An administrative penalty will be recommended as a result of this citation, per HRC 42.078. You must have the background checks renewed and you must receive the letter telling you that you are eligible prior to returning to the operation.
Some of the staff records were still inaccessible today. I was not able to access the training records for 4 staff members.
