During the course of a DFPS investigation it was found that an infant was asleep in a restrictive device.

A caregiver used poor judgement by pouring hot water into a sink with exposed metal pipes, resulting in an infant receiving a second degree burn when the infant touched the metal pipe.

At the time of the incident the caregiver was unaware of the location of the infant. The caregiver believed the infant was still near the toy bin in the middle of the room.

The operation failed to timely provide first aid to a second-degree burn. Additionally, the operation used unidentified ointment on the infants burn.

An infant was able to access a staff only area that had an exposed metal pipe. The infant was able to crawl in-between the infant area and the staff only area multiple times.

A caregiver was unaware of how any children were in her care. This was corrected when caregiver used their name to face (Pro care) to verify the children in their care.

During an investigation it was found that a caregiver did not utilize good judgment when they placed a pacier from one child into the mouth of another child. The operation has taken action to ensure proper training of staff and compliance with the Minimum Standards as it relates to caregiver responsibilities.
