Based on information obtained during an investigation, it was determined that a child was left unattended on the playground.
The first aid kit contained antiseptic that expired 11/2024, alcohol pads that expired 4/2025, and anti-itch spray that expired 5/2024. This was corrected at inspection when the director threw the expired first aid kit items away and other non-expired antiseptic was available.
Out of ten children records reviewed, one child was missing proof of 1 of 4 Dtap, 1 of 3 IPV, and 1 of 4 PCV immunizations.
A child was observed in a high chair that had the safety straps removed. This was corrected at inspection when the child was removed from the highchair, the highchair was removed from the classroom, and safety straps were ordered during inspection.
Two children medically diagnosed with a food allergy had food allergy emergency plans on file for review. The food allergy emergency plans were not signed and dated by a parent.
A caregiver was observed to have contact with children in a classroom before the operation recieved notification of eligibility. This was corrected when the caregiver left the operation.
