A video was reviewed pertaining to the incident that occurred on 7/21/2021. One caregiver was supervising five children in the sandbox, while one child was observed kicking and standing on another child's back. Another child was playing in the rock area unsupervised. The second caregiver was inside the classroom with another.
The documentation for the monthly inspections for the fire extinguisher, smoke alarm, and carbon monoxide detector were not complete.
Two caregivers were present without active background checks. Both caregivers left during the inspection.
