Ten personnel records were reviewed. The following were missing: Five out ten files were missing orientation documentation Fiive out of ten files were missing documentation of education requirments. Three out of ten files were missing annual training hours. One staff was missing 13 hours, the second staff was missing 12 hours, and the third staff was missing 24 hours. Four out of ten files were missing preservice training.
During the inspection a caregiver was observed in the classroom responsible for children without a cleared background check. This was corrected by the caregiver leaving the premises.
Out of six personnel records reviewed, the following was observed: Five records were missing proof of orientation completion; Four records were missing proof of background checks; Two records were missing proof of SIDS/Shaken Baby Syndrome training; and, Two records were missing Proof of Education and a Photo ID.
Out of ten children's records reviewed, one record was missing an emergency contact address.
A review of the Employees Who Left Employment in 2021 data from February 23, 2022, revealed that the center had not reported the number of employees who left employment in 2021 by February 22, 2022, as required.
