For his last completed training year, the director was missing some of the annual required training topics related to health and safety.

The operation did not annually certify in writing that they have reviewed each of the recall notices issued by the CPSC and that there are no unsafe products in the center. This was corrected at inspection when the director updated and certified the product safety recall form.
