A child was fed the incorrect bottle.
It was found that a caregiver hit a child in care on the back of their neck.
Based on the information obtained, it was determined that a child was unknowingly left outside of the CDC?s playground door while his class transitioned back to their classroom. This was corrected when an elementary school teacher, who was on the older children playground, found the child and assisted him back to his caregivers.
A child was able to leave his classroom and walk in the hallway without the knowledge of his caregivers. A caregiver from a different classroom found the child in the hallway and returned him to his class.
Based on the information obtained, it was determined that a child was left unattended in the classroom for roughly 30 minutes. The shool corrected this by providing additional training to all caregivers and using a face to name chart during each transitioning period.
10 caregivers were missing the required training topics in food allergies, 3 caregivers were missing training in hazardous materials.
A child with a food allergy does not have a medication listed on the allergy action plan on site.
