Observed an infant under 6 months old sitting in a bouncy seat while the caregiver held a bottle to the infants mouth. This was corrected when the caregiver was explained the minimum standards for feeding infants under 6 months old.

746.2415(b) - AP Cribs - Only Snug Fitting Sheet and Crib Mattress Cover Allowed if Infant Younger than 12 Months of Age · High

Observed a 2 month old infant sleeping in a crib with a blanket covering the infants body from waist down. This was corrected when the blanket was removed from the crib. An administrative penalty will be recommended as a result of this citation, per HRC ?42.078.

746.1205(a)(4) - Supervision ensures each child?s safety, well-being, including physical proximity and auditory or visual awareness of each child?s on going activity · High

A child was left outside on the playground without caregiver supervision for approximately 2 minutes. This was corrected when the child was found and brought back into the operation.

746.307(b)(4) - Parental Communication - Situation that Placed a Child at Risk · High

The parent of a child who was left outside by themselves was not notified immediately after the incident. The parent was notified of the incident the following day. I spoke with the operation about notifying the parent immediately of the incident and to not wait.

Observed two rooms with bottles and cups not labled with the child's name and last initial. This was corrected when the bottles were labeled.

746.605(11) - Required Admission Information - Emergency Medical Authorization · High

Observed 8 children files and one file was missing the Authorization for medication attention.

746.4907(a)(1) - Installation and Maintenance of Loose-fill Surfacing - Six Inches if Equipment 5 Ft or Less · Medium High

Observed two slides on the playground without any loose fill. This was corrected when the slides were removed from the playground during inspection.

Based on the preponderance of evidence a child was able to leave an operation while outside on the playground.

746.3407 - Maintenance of Building, Grounds and Equipment · Medium High

Based on the preponderance of evidence, it was determined due to a hole in a fence, a child was able to get out of the playground.

746.1201(4) - Responsibilities of Employees and Caregivers -Ensure No Child Abused, Neglected, or Exploited · High · No longer displayed by the state

Based on the information obtained during the investigation, a child was left unattended outside on the playground for approximately 5-10 minutes.
