Information obtained throughout the investigation shows that the operation failed to report a situation that placed a child at risk to Child Care Regulation within 48 hours of when the incident occurred.

Information obtained throughout the investigation shows that the operation failed to obtain a parent signature and provide a copy of the incident report with parents within 2 days of when the incident occurred.

Upon naptime there were one child that was laying in front of exit door. This was corrected onsite when the caregiver moved the child out from the exit door.

Upon observatation there were a total of nine mats that needs replacements. The nine mats had ripps and tear that displayed the form inside.
