It was found that the operation is open, with children from 3:15 until 6:00pm Monday- Friday during the school year and the director is present at the operation Monday-Friday at around 5:20 until 6:00pm, which is less then 75% of the operating times.

During a review of the children files, there were 4 children who did not have the emergency medicall attention completed.

Observed another caregivers file that did not have the notarized affidavit or the pre-employment affidavit. There was also no proof of education, photo ID. Both files only had 8 hours of pre service training.
