It was found that an infant was laid down in their crib with an iPad equipped with a shoulder strap that presented a choking hazard.
It was found that a child was left alone in a classroom during the transition to the outdoor play area.
During the investigation it was found that a child was placed in an unlocked room without a caregiver. The caregiver remained on the outside of the closed door.
