A child was able to leave the classroom and go to the hallway next to the classroom to get their lunchbox without the caregivers knowledge. The child was in the hallway around 15-30 seconds before another caregiver found the child and took the child back to the classroom.
After going through 8 staff files, one caregiver had the affidavit form in their file but it was not notarized.
Two caregivers were present at the operation without the operation receiving their background check eligibility notification letters. The caregivers left the operation during the inspection.
