The caregivers did not know how many children they had in care. When asked, they told me 15. When we counted together, it was found that there were 13.

Children came in from swimming, and went to right to playing. The children were brought to the carpet after about 10 minutes, and were broken into groups to change into dry clothes. No mention of washing hands was made. When I asked one child if he was going to wash his hands, he went and washed them but there were no paper towels in the men's room, so I went into the lady's room, there were no paper towels or hand dryers there either.

A caregiver was alone with children, despite having conditions on his background check stating that they cannot be alone with them or transport children due to their provisional status. HHSC will recommend an administrative penalty as a result of this citation, per HRC Section 42.078.

The use zones for the climbing structure and the swings do not have an acceptable depth of uncompressed loose fill surfacing. There is a lot of grass growing up through the surfacing that has compacted a lot of it.
