Sheets did not fit snugly in cribs and were bunched around the sleeping baby. There was space between the mattress and the crib base that was accessible to an infant. The crib mattress did not fit tightly in the crib. Corners of the mattress were curling up. A pacifier cord was attached to the baby while sleeping in the crib. The cord was removed at inspection.
An evacuation diagram was not posted in one classroom
There is an active play structure on the playground that does not have adequate use zones. One of the sides of the equipment is less than 4 feet from the edge of the wooden footer at the edge of the playground.
The smaller play structure on the playground requires loose fill as it is over 20 inches tall.
