Several caregivers were missing required annual trainings for working with infants and children. Five out of eleven were missing SIDS; Seven out of eleven are missing CPR/First Aid; Four out of eleven are missing Employee preparedness plan; Three out of eleven are missing Blood pathogen training and One out of eleven is missing Orientation.

746.4751(c) - Maintenance Procedures for Active Play - Hazards Removed or Repaired · Medium High

During the inspection several holes that were approximately five inches wide were observed at the bottom of the exterior fence. There was a large nail sticking out of one of the railroad ties on the ground of the play area. The nail was removed from the border during the inspection. There was one holes were observed under one of the railroad ties where the ground had eroded.

A caregiver used poor judgement when they picked up a cot with a child on it, moved it to another area, and then let it drop to the floor.

It was found during the inspection three children were in high chairs and were not strapped in. This was corrected at the inspection when the children were strapped in and the chairs with broken straps were removed.

It was found during the investigation a caregiver wiped a child's face and nose and did not wash their hands after.
