ALLEGATION(S): Facility is overcapacity INVESTIGATION FINDINGS: On 03/27/2025 at 12:00 PM, Licensing Program Analyst (LPA) Kate Huang conducted an unannounced complaint visit to deliver investigation findings regarding the above allegation. LPA met with the licensee, Yonghui Hu, and explained the purpose of the visit. During today's inspection, LPA toured the facility, both indoors and outdoors, and observed the child-to-staff ratios. A total of 9 children (2 infants and 7 preschool-age) and 3 staff members (the Licensee and two assistants) were present. Prior to this inspection, on 01/31/2025, LPA did not observe any violations of the child-to-staff ratio during an unannounced initial complaint investigation at the facility. On that date, LPA observed 8 children (3 infants and 5 preschool-age) and 2 staff members. LPA interviewed staff and reviewed the children’s roster during both inspections. Both the provider and the assistant stated that they never operated the facility out of ratio. Unsubstantiated Estimated Days of Completion:
ALLEGATION(S): Facility operating out of ratio INVESTIGATION FINDINGS: On 11/05/2024 at 1:30pm, Licensing Program Analysts (LPAs), Farida Raja and Linke ‘Kate’ Huang conducted an unannounced complaint visit to deliver investigation findings for the above allegation. LPAs met with Licensee, Yonghui Hu and explained the purpose of today's visit. During today's inspection, LPAs toured the facility both indoors and outdoors and observed ratios. LPAs observed a total of 7 (2 infants and 5 preschool age) children and 2 staff. During the course of this investigation, LPAs interviewed Licensee, staff and parents and reviewed relevant records. Based on interviews, Licensee and staff stated that they have had a total of 5 to 7 children and have not had over 4 infants in care. Based on parent interviews, parents stated that they have not observed facility operating out of ratio. Continued on Page 2 Unsubstantiated Estimated Days of Completion:
A licensed child day care facility shall provide to the parents or guardians of each child receiving services in the facility copies of any licensing report that documents any Type A citation that represents an immediate risk to the health, safety, or personal rights of children in care as set forth in paragraph (1) of subdivision (a) of Section 1596.893b. This requirement is not met as evidenced by: During Licensee interview, LPAs observed the Acknowledgment of Receipt of Licensing Reports (LIC 9224) form signed by parents but Licensee did not provide parent's with a copy of the report with the Type A citation dated 10/07/2024. This poses a potential risk to the health, safety and personal rights of children in care.
102417 (g) (8) Each family child care home shall have a current roster of children as specified in Health and Safety Code Section 1596.841. This requirement is not met as evidenced by: Based on observation, record review and interview, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. Licensee did not have a current roster during today's inspection.
102419 (d) (1) The licensee shall request the child's parent or authorized representative to sign and date the bottom portion of the notice form LIC 995A (8/06), which acknowledges that the parent or authorized representative has received and read the LIC 995A. The bottom portion of this form must be kept in the child’s file as proof that the parent or authorized representative has been notified of his or her rights and received a copy of the Caregiver Background Check Process, LIC 995E (6/05), and the Family Child Care Consumer Awareness Information, LIC 9212 (10/05). This requirement is not met as evidenced by: Based on observation, record review and interview, the licensee did not comply with the section cited above for two chidlren (C1 and C2) which poses a potential health, safety or personal rights risk to persons in care.

This poses an immediate risk to the health, safety and perosonal rights of chidlren in care.
102425 Infant Safe Sleep (j) (D) Documentation shall be maintained in the infant’s file and be available to the Department for review. Documentation shall include the following: c. This requirement is not met as evidenced by: Through file review, LPAs observed that Licensee is not conducting 15 minute nap checks and documenting them for two infants, C3 and C4.
