101226 Health-Related Services: (d) The licensee shall maintain the following first-aid supplies in a location accessible to staff but inaccessible to children; (1) A current edition of a first-aid manual, (2) Sterile first-aid dressing, (3) Bandages or roller badanges, (4) Adhesive tape, (5) Scissors, (6) Tweezers, (7) Thermometer, (8) Antiseptic solution. This requirement is not met as evidenced by: Based on observation, the licensee did not comply with the section cited above as the facility does not have a complete and full first-aid kit that inclues all of the items listed in the regulation above which poses/posed a potential health, safety or personal rights risk to persons in care.
(1) On or before March 30, 2018, a person who, on January 1, 2018, is a licensed child day care provider, administrator, or employee of a licensed child day care facility shall complete the mandated reporter training provided pursuant to paragraphs (2) and (3) of subdivision (a), and shall complete renewal mandated reporter training every two years following the date on which he or she completed the initial mandated reporter training. This requirement is not met as evidenced by: Based on record review, the licensee did not comply with the section cited above as the correct mandated reporter trainings were not completed or accompanied with a current waiver which poses/posed a potential health, safety or personal rights risk to persons in care.
(d) The child care center director, or the substitute director as specified in (f) below, shall be on the premises during the hours the center is in operation. This requirement is not met as evidenced by: N/A
(d) All personnel records shall be maintained at the child care center and shall be available to the licensing agency for review. This requirement is not met as evidenced by: Based on interview & record review, the licensee did not comply with the section cited above as one staff member does not have a personnel record on file for review which poses/posed a potential health, safety or personal rights risk to persons in care.
(a) The licensee shall ensure that a separate, complete and current record for each child is maintained in the child care center. This requirement is not met as evidenced by: Based on record review, the licensee did not comply with the section cited above as several children's files were either incorrectly or incompletely filled out with required information missing which poses/posed a potential health, safety or personal rights risk to persons in care.
(b) The name of the child care center director, and any fully qualified teacher(s) designated to act in the child care center director's absence, shall be reported to the Department within 10 days of a change of child care center director or designee(s). This requirement is not met as evidenced by: Based on interview, the licensee did not comply with the section cited above as the facility director has been changed as of early August 2023 and CCLD was not norified of this change which poses/posed a potential health, safety or personal rights risk to persons in care.
