ALLEGATION(S): Staff did not ensure they were not out of ratio INVESTIGATION FINDINGS: On March 12, 2026, at approximately 2:35pm, Licensing Program Analyst (LPA) Maria Olguin-Leon conducted an unannounced visit to deliver complaint findings for the above allegation. LPA met with Site Supervisor, Michelle Salinas Quintanilla and explained the purpose of the visit. Present during today’s visit was Site Supervisor and 3 staff caring for 13 children (4 toddlers and 9 preschool). Facility is operating within capacity. During the course of the investigation, LPA conducted both staff and parent interviews, conducted observations and reviewed pertinent documentation that was received. LPA determined that based on the information obtained there is not sufficient evidence to prove that Staff did not ensure they were not out of ratio. Although the allegation above may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore, the above allegations re UNSUBSTANTIATED. Upon receipt of this report, Licensee shall post the Notice of Site Visit. Notice of Site Visit shall be posted for 30 consecutive days. Failure to maintain postings as required will result in an immediate $100 civil penalty. This report is public and can be reviewed. 101217 Personnel Records (a) The licensee shall ensure that personnel records are maintained on the licensee, administrator and each employee. Each personnel record shall contain the following information: (11) A health screening as specified in Section 101216(g). This requirement was not met as evidenced by: Based on observation, interview and record review, the licensee did not comply with the section cited above as S1, S2, and S3 do not have health screening on file, which poses a potential health, safety or personal rights risk to persons in care.
California Lead Action Level at Child Care Centers (b)(1) A result with values of 5.5 ppb or greater shall be deemed an Action Level Exceedance. This requirement is not met as evidenced by: Based on results received from the certified water samplers, the center exceeded 5.4 ppb of lead in their water source. Once received, the center took immediate and proper actions to remediate the fixtures. This poses a potential health, safety, or personal rights risk to children in care.
